We’re here to help you direct your own care.
We believe in person-centered care, provided by staff who live in the communities where they work and who understand the unique needs of clients and their caregivers.
Our tools and services are designed with people in mind. We serve clients and their caregivers through Medicaid programs across the United States. We work with state agencies, managed care organizations, case managers, caregivers, and clients.

We serve clients and their caregivers. Our clients include older adults and people of all ages with disabilities and other long-term care needs.

Certified Employee-Owned Company
In 2025, Consumer Direct Care Network (CDCN) became a Certified Employee Owned company. More than 30% of the company is now dedicated to employee ownership through the formation of an Employee Ownership Trust (EOT). This structure ensures eligible employees and caregivers are central to the company’s growth and success.
The EOT supports several CDCN key goals:
- Recognizes employees as essential partners by valuing their contributions and including them in the company’s future success.
- Protects the mission and integrity of care by ensuring that company decisions remain focused on people over profit.
- Secures long-term sustainability through a thoughtful ownership transition that strengthens CDCN in a competitive care landscape.

Consumer Direct Care Network’s executive team is deeply committed to our mission, vision, and values. With extensive experience in self-direction programs, our leaders bring knowledge and expertise to their roles. Many of them started their careers working directly with the people we serve, providing them with a strong understanding and empathy that guides our company every day.

Where we come from
Consumer Direct Care Network was founded in 1990 in Missoula, Montana, to provide in-home nursing care. In 1998, self-directed services became an option in Montana. We saw firsthand that people are happiest and healthiest when they receive the care they need in their homes and communities. We began our journey to support programs that allow for greater choice and control for Medicaid participants across the country.
Where we are today
Since then, we have expanded our service models and our range. We support a variety of programs in 14 states across the country.
Our person-centered approach, focus on community-based offices and staff, comprehensive experience, innovative technology solutions, and collaborative relationships make us a leader in the field of self-direction.
We continue to stand upon our caregiving foundation established more than 30 years ago: empowering and supporting people to live the life they want and remain in their homes and communities.

We believe in the philosophy of self-direction. We know people are happiest and healthiest when they have control over their care. Self-direction will always be at the heart of what we do. We will continue to advance self-direction, providing high-quality services to our clients and their caregivers and working to empower and support people to live the life they choose.
