To help keep the individuals and families we serve and our staff healthy and safe during the global COVID-19 outbreak, Consumer Direct Care Network is asking many of our employees to work remotely. For assistance, please contact your assigned CDCN staff. If fax or email is your preferred way to turn in paperwork, you may continue to fax or email your CDCN office. You will find a list of CDCN office emails listed by state at the bottom of this post.

We are committed to making public health our priority and recognize the impact this will have across the country. We are working directly with state agencies and health plans to find the best ways to maintain continuity of care.
