Home Health and Hospice of Montana is pleased to announce we will begin seeing hospice patients on January 4, 2016.  Adding hospice care to our Home Health line of service allows for a continuum of care which benefits our patients and their families, especially at the end of their life.

Hospice care is provided to people facing a life limiting illness. Hospice is not place, nor does it hasten death. Hospice goals are to help people LIVE until they die. We are able to offer care in the most intimate of settings and provide a safe, secure net for patients and their families.

Hospice care is considered to be the model for quality and compassionate care. It involves a team approach and at the center are patient and family. Hospice care encompasses emotional, physical and spiritual care. Our team consists of physicians, nurses, social workers, clergy, certified nursing assistants, physical therapist, occupational therapists, speech therapists, dieticians, and volunteers and music thanatologists.

Did you know pain can be physical, emotional or spiritual? Our interdisciplinary team is available to help navigate this concept with the patient and their families allowing us to provide the best care in pain management.

Does the patient have a goal, unfinished project or desires to reconnect with a family member? Hospice can assist patients and families with their wishes.

Hospice is a wonderful addition to the Consumer Direct Care Network and we look forward to serving our community.
